Referrals

Referrals are only accepted from healthcare providers.

All referrals should be sent to Thrombosis Central Access & Triage office via Connect Care or Via Fax at (403) 270-8453. The Triage phone number is: (403) 955-8657

Referral Form

Thrombosis clinics are held at many sites throughout the Calgary zone that includes Calgary Thrombosis Consultants as well as clinics at the Richmond Road Diagnostic & Treatment Centre (RRDTC), South Health Campus (SHC) and Internal Medical Associates (Westbrook).

You may indicate your preferred location on the referral and the  Central Triage office will try to accommodate if appropriate.

Our clinic will contact patients directly to confirm appointment details if they are directed to our office.

Please have the patient stay on their anticoagulant until they are seen in the Thrombosis Clinic, if relevant.

Thrombosis Central Access & Triage Estimated Wait Times:

• Urgent referrals: 1-3 weeks.
• Semi-Urgent referrals: 4-12 weeks.
• Routine referrals: 3-6 months.

All referrals are triaged based on acuity. Wait times may vary based on referral volume.

If there is any concern for worsening symptoms, then the patient should seek urgent medical attention. If the patient is being referred from the emergency department, then follow-up with their family doctor is also recommended.
